Parents and Carers

Thank you for considering us at a time we know can be stressful and difficult for you and your family. In order for the best possible care for your child we need a formal referral from your General Practitioner, Paediatrician or Paediatric Specialist who will co-ordinate all of his/her investigations and deliver your clinical care. Please ask your clinical specialist to refer via The Portland Hospital for Women and Children, or The Cromwell Hospital.

Referral Forms

Referrals may be made on your Practice headed paper, the in-house forms available from The Portland Hospital for Women and Children, or The Cromwell Hospital and sent to the relevant department.
